Audience: Tobacco Control professionals working with health systems change for patient brief interventions. Key Points: The poster will demonstrate how a health systems change pilot project innovatively referred patients for telephonic counseling and received patient disposition reports back from the counselors. * A Community Health Access Network (CHAN) of 10 Federally Qualified Health Centers across the state pilot tested an evidence-based tobacco cessation intervention utilizing the NH Tobacco Helpline proactive telephonic counseling service, "QuitWorks-NH in one of the member community health centers. * Technological changes to the EMR system allowed referrals through electronic fax to QuitWorks-NH. Technological changes to CHAN's electronic lab report receiving system allowed the return of a referred patient's status for accepting services (or not) through a secure health information exchange network (HIE). * This successful pilot project has been interfaced with two additional health centers; the goal is to bring all 10 CHAN sites into the electronic exchange flow. Educational Experience: The audience will have the opportunity to view the poster showing each step in this year-long health systems change process as well as take a hand-out away from the session that maps the entire process. Benefits: Benefits to the audience include observing one process that resulted in a successful health systems change that also met meaningful use requirements. This poster session is relevant because it supports the evidence-based strategy that consistently, all healthcare providers should identify patients that use tobacco, advise and assist them to quit using and arrange follow up from the brief interventions.
