1. To operationalize a model for “best practices” tobacco cessation treatment
Based on the PHS Guidelines of 2000 ,HMHP’s Regional Tobacco Treatment Center has provided “best practice” tobacco cessation since 2007 in 2 hospital-based settings. The program provides medication in combination with behavioral counseling to improve clients’chances of quitting. In the past 2 fiscal years, the end of treatment quit rate was 94% .
Critical to the success of the program have been the following elements:
1. networking with community groups such as health departments, community agencies including those that target minority populations ;
2. training hospital residents regarding the use of the 5 A’s and an R and the implementation of a Fast FAX referral system to expedite physician referrals to the program ;
3. identifying inpatients who use tobacco products at the time of admission and providing bedside intervention for inpatients the first 2 years of the program and
4. marketing through different media to increase community awareness of program services.
5. developing relationships with local employers and businesses to assist in developing policies around tobacco-free worksites .
The presentation will outline program services as well as the outcomes of surveys conducted with program graduates in the past 4 years.
